As an Epidemiologic bridge between data collection in public health surveillance
and disease intervention programs/services (disease control and prevention), the
Model Collaboration Center coordinates the integration of the Public Health
Agency's "disease intervention epidemiology"
support for HIV(& related
STD, TB & Viral Hepatitis) prevention and care programs (disease
interventions/services) planning, development, implementation, service
delivery/utilization monitoring, outcome evaluation and capacity development.

Model Collaboration Center at the Public Health Agency level is a
model for translating these guidelines' and providing disease
